Promo Mechanics

1. Myra E VitaWhite Whitening Face Lotion “Be A VitaWhite Stunning Beauty” promo is open to all women ages 18-35 who possess the stunning qualities that embody the brand and who would want to be glam it up in a TV commercial with Paula Taylor.
2. To join the promotion, contestants can either:
   1. Upload pictures of themselves with the Myra E VitaWhite tube in the Myra E VitaWhite Website (http://www.myravitawhite.com), enter their name, address, telephone/mobile number and tell us how Myra E VitaWhite has changed their life.
   2. Write their name, address, telephone/mobile number, and signature on a piece of paper and enclose three (3) wallet size pictures of themselves with the Myra E VitaWhite. They should also tell us how Myra E VitaWhite has changed their life.

3. Entries will be accepted from July 1, 2009 until August 31, 2009 only.
4. Fifteen (15) finalists will be selected by the Myra team on September 1, 2009. The finalists will be informed through email, text message or telephone and will be called for a face to face interview to be scheduled anytime from September 1 to 10, 2009. Criteria for judging as follows:

   Healthy White Beautiful Skin – 70%
   How Myra E VitaWhite has changed their life – 30%

5. Three (3) winners will be selected on September 15, 2009. The winners will receive P100,000 (tax free) each.

6. Winners will be notified via phone call, to be followed by registered mail and/or email instructing her on how to claim the prize and the invitation to be part of the tv commercial. Indicated in the letter will be the date and time she would be able to claim her prize and the corresponding tv commercial shoot, fitting and make up test schedules.
7. Winners who will not available, or wishes not to be part of the tv commercial will be disqualified.
8. Cash prize tax and all applicable taxes shall be shouldered by the Client.  Cash prizes are non transferable.
9. Ten (10) People’s Choice winners will also be named after the promo contest.
10. The ten (10) People’s Choice winners will be determined through an online voting system.
11. Those who wish to vote would type in their name, age, address and/or email address in the website together with their contestant choice.
12. The ten (10) most voted contestants will be named as people’s choice. Winners will receive Myra E Gift Packs. To claim the gift pack, contestants will be notified via phone call and instructions on how to receive the gift pack will be given.
13. By participating in this promo, participants agree to be bound by the promo rules and regulations and decisions of Innovitelle, Inc. and to participate in the promo advertisement without additional consideration. To the extent allowed by law, acceptance of the prizes constitutes permission for Innovitelle, Inc. to publish winners’ names and/or show the winners’ tv commercial without prior notice and without additional consideration.
14. Employees of United Laboratories, Inc. and those of its affiliates, subsidiaries, distributors, dealers, suppliers, advertising and promotional agencies, DOH-BFAD representatives, and their relatives up to the 2nd degree of consanguinity or affinity, are not qualified to join this promotion.
15. If the winner fails to claim her prize within 60 days from his/her receipt of the notice, the prize shall be forfeited in favor of United Laboratories, Inc. with prior approval of DOH/BFAD.

Prizes & No. of Winners

Prize will be P100,000 (net of tax). Three (3) winners will be selected from the contest.

Notification/Verification of Winners

The winners will be notified via registered mail. This will instruct him/her how to claim the prize. Indicated will be the date and time he/she would be able to claim his/her prize.

Redemption of Prizes

1. Winners must follow the date and time indicated in her registered mail/email as to the schedule of claiming of the prize.
2. To claim her cash prize, the winner must present (2) two valid ID’s with photo (Passport, Company ID, Driver’s License, SSS ID, School ID, Postal ID, Voter’s ID) and the letter of notification sent to them.
3. If the winner fails to claim his/her prize within 60 days from his/her receipt of the notice, the prize shall be forfeited in favor of United Laboratories, Inc. with prior approval of DOH/BFAD.

Terms & Conditions

1. TERMS & CONDITIONS/DISCLAIMER WHEN CONTESTANTS UPLOAD THEIR PHOTOS
   1. All uploaded image shall be first subjected to a pre-screening process by Innovitelle, Inc. and its site administrators before actual inclusion in the “Be A VitaWhite Stunning Beauty” site
   2. Innovitelle, Inc. reserves the right to deny / grant approval to any uploaded photos. Likewise, photos may be removed without prior consent or notification
   3. Uploading copyrighted or defamatory images, or images other than the contestant’s will result in immediate disqualification from the contest
   4. Innovitelle, Inc assumes no responsibility for damages that may arise from the use and misuse that may arise from the public display of contestants’ images
   5. All photos and entries become property of Innovitelle, Inc. The   company may use said property in any of its promotional or advertising activities without further consent of contestant. These activities may include print and television advertisements, in-store collaterals and brochures.
